Accidents happen, but the CSEA Group TermPLUS Plan can help you protect your family during the unexpected. The CSEA Group TermPLUS Life Plan pays DOUBLE your benefit amount – up to $600,000.00, if you're killed in a covered accident. This benefit is exclusively available to CSEA members only.

Additional Benefits for Accidental Injuries

If you're in a covered accident and suffer specific injuries, the plan will pay you benefits based on your loss.

Consider the CSEA Group TermPLUS Life Plan for your life insurance needs. You are under no risk or obligation. Once you apply and are approved for coverage, you will be sent a Certificate of Insurance, including complete benefits and terms of the plan.

Look this information over. Then decide if you want it as ongoing protection for you and your family. If the plan is not what you had in mind, then you may cancel within 30 days of receiving your Certificate of Insurance and your premiums will be promptly refunded and your coverage invalidated, provided there have been no claims.

Benefits

WHO IS ELIGIBLE

CSEA members who are US residents, under age 65, employed less than 7 months and are continuously working at least 20 hours per week qualify for a minimum amount of coverage based on age and monthly salary. This minimum coverage amount is guaranteed and can be found in the rate tables provided. If enrolling after 7 months of employment or when enrolling for coverage in excess of your pre-approved amounts, evidence of insurability is required and you (and your spouse, if enrolling) must complete the full medical underwriting application.

DOUBLE BENEFITS FOR ACCIDENTAL DEATH - MEMBER ONLY BENEFIT

If you're an insured active employee and are in a fatal car or other accident causing death, your CSEA Group TermPLUS Life Plan will pay double your benefit amount up to $600,000.00. This is an important benefit when you consider the fact that accidents do happen. The extra cash could come in handy for your loved ones when they're faced with the sudden, unexpected loss. Covered losses and time frames are included in your Certificate of Insurance which also provides complete plan details and limitations.

ADDITIONAL BENEFITS FOR ACCIDENTAL INJURIES

If you're in a covered accident and suffer specific injuries, the plan will pay you benefits based on your loss.

  • Loss of Life: 100% of benefit ammount
  • Both hands, both feet, or the sight of both eyes: 100% of benefit amount
  • Two or more of the following: one hand, one foot, the sight of one eye: 100% of benefit amount
  • One hand, one foot, or the sight of one eye: 50% of benefit amount
  • Loss of sight, speech or hearing means total and permanent loss. Loss of limb means severance through or above the wrist or ankle. No more than one benefit, the largest benefit, is payable for all losses to the same limb due to or related to any one accident. No more than the Principal Sum is payable for all losses due to or related to any accident, except as otherwise provided under the What Benefit is Payable section on the AD&D Insurance Page.

SPOUSE AND CHILDREN COVERAGE AVAILABLE

Coverage for you is only half the story. If your spouse works or takes care of children at home, you need to consider the value of your spouse's contributions to your lifestyle.

With Group TermPLUS Life, your spouse under age 65 (lawful spouse, including insured's legally registered and valid domestic partnership) can enroll for half of your benefit amount and your children up to age 26 can also get coverage up to $10,000.

ACCELERATED DEATH BENEFIT

This valuable feature allows you to collect half of your benefit amount up to $75,000.00 at a time when you'll need it most. If you're diagnosed with a terminal illness and given a life expectancy of 12 months or less, you can access your cash benefit to use any way you see fit.

This added cash can be used to help pay medical bills … or to cover household expenses so a family member can stop working to care for you. The remainder of your benefit will then be paid to your beneficiary.

Please note that receipt of Accelerated Death Benefits may affect your eligibility for public assistance programs and may be taxable. Prior to applying to receive such benefits, you should consult with the appropriate social services agency and seek the advice of a qualified tax advisor.

WAIVER OF PREMIUM IF TOTALLY DISABLED

Your premiums will be waived if you're an active employee and qualify for total disability before your 60th birthday and remain disabled for four consecutive months. Premiums are due during the first four months of disability. Once approved, your benefits will continue and you won't owe any further payments as long as you remain totally disabled. You should always continue premium payments until your disability is approved.

COVERAGE DOESN'T END … EVEN IN RETIREMENT

The full benefit amount you select is yours to keep until age 65. At age 65, your benefit reduces by 50%. Then at age 70, your benefit reduces to $15,000.00, at which time you can keep this coverage for as long as you need it. (Your spouse and/or child coverage reduces to $5,000.00 when you, the member, reach age 65.)

Your protection will never end unless you are no longer an employee, you end your CSEA membership, you fail to pay your premiums, or the group policy is terminated.

When you truly retire, you can continue your life insurance; you receive monthly California Public Employees Retirement System (CalPERS) benefits from which premiums may be deducted; you continue your CSEA membership; and you send in an application requesting benefits to be continued into retirement before your retirement date and continue premium payments thereafter.

GUARANTEED COVERAGE

If you're a new employee and enroll within seven months of employment, you are guaranteed the minimum amount of coverage based on your salary, regardless of health. Your spouse is also guaranteed $20,000.00 of coverage if enrolling within seven months of your employment. If enrolling after seven months of employment or when enrolling for coverage in excess of your pre-approved amounts, evidence of insurability is required and you (and spouse, if enrolling) must complete the application for insurance.

NO WAITING PERIOD

Full benefits are payable on the very first day your coverage takes effect.

CONVENIENT PAYROLL DEDUCTION*

As a CSEA member, you'll never have to worry about paying your premiums on time. That's because your premiums are paid through the convenience of payroll deduction*. If you are retired, your premiums will be deducted from CalPERS.

WORK JUST 20 HOURS A WEEK

You're eligible for coverage if you are a CSEA member, actively employed working at least 20 hours per week (excluding Retired Members). Retired CSEA members are still eligible for coverage provided they are under the age of 65 and remain a Retired CSEA member. Evidence of insurability is required.

You must not be on active duty with the military.

If you decide your Endorsed CSEA Group TermPLUS Life Plan isn't for you, just let us know within the first 30 days. We'll promptly refund your premiums and invalidate your coverage provided there have been no claims … no questions. No obligation.

* Payroll deduction is only available for SEIU, CSUEU, ACSS, and Retirees. All other groups leverage auto-renewal through automatic bank deduction or credit card.

Once a member reaches age 65, coverage is reduced by 50%. At age 70, member coverage is reduced to $15,000. Spouse coverage may be purchased for up to 50% of member coverage. Child coverage may be purchased for up to $10,000. When member turns 65, spouse/child coverage will be reduced to $5,000.

To verify benefit amounts applied for, please include a copy of your pay stub dated within 30 days of your signed application.

Terms

Effective Date of Coverage

Your CSEA Group TermPLUS Life benefits can begin following approval from New York Life. Your coverage will take effect on the first day of the month after your application has been approved and first payment has been received, via payroll deduction or from CalPERS if you are retired. Once approved you will receive a Certificate of Insurance describing the terms and conditions of your coverage. If you are not working full-time on the date your insurance is scheduled to become effective, your insurance will not become effective until you return to full-time work. However, if you are not actively working due to a non-working day, you will still become insured on the scheduled effective date provided you would have been able to do your regular job on that non-working day.

When Coverage Ends

Your coverage will end upon Policy termination age as long as you continue to pay premiums when due, remain a CSEA member, and the Policy remains enforced. In addition, dependent coverage will terminate when the dependent spouse or child ceases to be an eligible dependent

Beneficiary Designation

You can name any beneficiary you wish for member coverage. You may change this beneficiary at any time, by written request. Your beneficiary is the person(s) last designated by you in writing, and recorded by or on behalf of New York Life Insurance Company. You are the automatic beneficiary for dependent insurance, as described in the Certificate of Insurance. If you wish to name another beneficiary for dependent spouse or child insurance, contact the administrator at +1-800-296-2732 for the applicable form.

AD&D Coverage Exclusions (Member-Only Coverage)

The accidental death and dismemberment benefit does not cover loss resulting from: intentionally self-inflicted injury, suicide or attempted suicide, while sane or insane; Occurs during, is due to or related to the insured member's duty in the military, naval or air services of any country; participation in or incarceration resulting from any of the following, other than as a victim, commission of a felony, illegal occupation or activity, an insurrection or terrorist activity; injury sustained while riding in or on, fall from or decent from any aircraft while in flight unless the insured is traveling solely as a passenger; sickness or disease or medical or surgical treatment of sickness or disease, except infections which occur as the result of an accidental bodily injury; or accidental ingestion of contaminated material, any medical, dental or surgical treatment unrelated to the accident, engagement in any of the following in a role other than as a victim: war, an act of war, or an armed conflict which involves the armed forces of one or more countries, or voluntary entry into the body of an intoxicant, excitant, hallucinogen or any narcotic or other drug or any gas, poison or poisonous substance.

How New York Life Obtains Information and Underwrites Your Request for Group TermPLUS Life Insurance  

In this notice, references to “you” and “your” include any person proposed for insurance.  Information regarding insurability will be treated as confidential.  In considering whether the person(s) in your request for insurance qualify for insurance, we will rely on the medical information you provide, and on the information you AUTHORIZE us to obtain from your physician, other medical practitioners and facilities, other insurance companies to which you have applied for insurance and MIB, Inc. (“MIB”).  MIB is a not-for-profit organization of insurance companies, which operates an information exchange on behalf of its members.  If you apply for life or health insurance coverage or a claim for benefits is submitted to an MIB member company, medical or non-medical information may be given to MIB, and such information may then be furnished by MIB, upon request, to a member company.

Your AUTHORIZATION may be used for a period of 24 months from the date you signed the application for insurance, unless sooner revoked.  The AUTHORIZATION may be revoked at any time by notifying New York Life in writing at the address provided.  Your revocation will not be effective to the extent New York Life or any other person already has disclosed or collected information or taken other action in reliance on it, or to the extent that New York Life has a legal right to contest a claim under an insurance certificate or the certificate itself.  The information New York Life obtains through your AUTHORIZATION may become subject to further disclosure.  For example, New York Life may be required to provide it to insurance, regulatory or other government agencies.  In this case, the information may no longer be protected by the rules governing your AUTHORIZATION.

MIB and other insurance companies may also furnish New York Life, its subsidiaries or the Plan Administrator with non-medical information (such as driving records, past convictions, hazardous sport or aviation activity, use of alcohol or drugs, and other applications for insurance). The information provided may include information that may predate the time frame stated on the medical questions section, if any, on this application. This information may be used during the underwriting and claims processes, where permitted by law.

New York Life may release this information to the Plan Administrator, other insurance companies to which you may apply for life and health insurance, or to which a claim for benefits may be submitted and to others whom you authorize in writing, however, this will not be done in connection with test results concerning Acquired Immune Deficiency Syndrome (AIDS) or Human Immunodeficiency Virus (HIV).  We may also make a brief report of your protected health information to MIB, but we will not disclose our underwriting decision

New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law.  Information in our files may be seen by New York Life and Plan Administrator employees, but only on a "need to know" basis in considering your request.  Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved.

If we cannot provide the coverage you requested, we will tell you why.  If you feel our information is inaccurate, you will be given a chance to correct or complete the information in our files.  Upon written request to New York Life or MIB, you will be provided with non-medical information.  Generally, medical information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured.  Your request is handled in accordance with the Federal Fair Credit Reporting Act procedures.  If you question the accuracy of the information provided by MIB, you may contact MIB and seek a correction.  MIB's information office is: MIB, Inc., 50 Braintree Hill Park, Suite 400, Braintree, MA 02184-8734, telephone 866-692-6901 (TTY 866 346-3642).  For Canadian residents, the address is: MIB Information Office, 330 University Avenue, Suite 501, Toronto, Ontario, Canada M5G 1R7, telephone 416-597-0590.  Information for consumers about MIB may be obtained on its website at www.mib.com.

For NM Residents:  PROTECTED PERSONS 1 have a right of access to certain CONFIDENTIAL ABUSE INFORMATION 2 we maintain in our files and they may choose to receive such information directly.  You have the right to register as a PROTECTED PERSON by sending a signed request to the Administrator at the address listed on the application.  Please include your full name, date of birth and address.
